Background: Cavernous sinus-orbital apex aspergillosis is a rare but serious complication and difficult to diagnose based on clinical radiological results. This condition frequently diagnosed at late stage because of its nonspecific varying symptomatology, specifically in immuno-supressive patients. Early diagnosis treatment the key prevent more central nerves system complications. Methods: We report case 80-year-old man with 2 month history retroorbital pain before he developed subacute cavernous sinus syndrome, 3th cranial nerve palsy right-sided painful ophthalmoplegia. Patient was immune suppression therapy for chronic lymphocytic leukemia. Neuroimaging including CT scan MRI suggested malignant tumor involving sphenoid extention ape. Results: The made trans-sphenoidal approach by histopathological examination. Soon after surgical drainage systemic anti-fungal drug therapy, Both resolved syndrome slowly start recover. Conclusions: emphasizes fact that invasive isolated must be considered list lesions causing particularly suppressed
